Hospital Discharge to Residential SUD Treatment: A Coordination Checklist

LifeSpring Recovery Admissions TeamJuly 2, 20265 min read

A practical checklist for discharge planners, case managers, and social workers coordinating a warm handoff from hospital to residential substance use treatment.

Why Discharge Coordination Matters

For patients with substance use disorders, the hours and days after a hospital discharge are among the highest-risk windows for relapse, overdose, and readmission. A warm handoff — where the patient moves directly from your care into a treatment program with no gap — is one of the most effective interventions available.

This checklist is built for the professionals who make those handoffs happen: discharge planners, ED case managers, hospital social workers, and care coordinators.

✅ Before You Call a Treatment Center

  • Confirm the clinical picture. Primary substance(s), last use, current withdrawal status, and any complicating medical conditions.
  • Gather insurance information. Payer name, member ID, and whether the plan is commercial, Medicaid (KanCare), or Medicare.
  • Pull recent records. H&P, medication list, labs if available, and any psychiatric evaluation. Most programs can work with partial records — do not let a missing document delay the call.
  • Assess patient willingness. A brief motivational conversation before discharge dramatically improves follow-through.

✅ During the Referral Call

  • Ask about bed availability and admission timeline. At LifeSpring Recovery, our admissions line is answered 24/7 and we can typically complete a clinical review the same day.
  • Verify level-of-care fit. Share the clinical picture openly — medical detox and residential treatment have different admission criteria.
  • Confirm insurance verification. A good admissions team will run benefits while you are still on the phone or shortly after.
  • Establish a single point of contact. Get a direct name and number for follow-up, and provide yours.

✅ At Discharge

  • Arrange transportation. A direct transfer is ideal. If family is driving, confirm the plan and the arrival window with the receiving facility.
  • Send records ahead. Fax or secure-send the discharge summary and medication list before the patient arrives.
  • Communicate medications clearly. Include anything administered in the last 24 hours — this is critical for safe detox management.
  • Close the loop. Ask the receiving facility to confirm arrival. At LifeSpring, we notify referring providers when a patient admits, with appropriate releases in place.
